New Patient Forms
- Ortho Form Packet – Provides basic patient details, dental/medical history, referral information, and a brief sleep screening questionnaire to help us better understand your health needs.
- Practice Info and Financial Policy Form – Outlines what to expect during your visits, including diagnostic procedures, appointment policies, emergency care, and financial responsibilities related to insurance and payment.
- HIPAA Consent Form – Grants consent for our practice to use and share your health information in accordance with HIPAA regulations.
- Failed Appointment Policy – Outlines our scheduling, cancellation, and rescheduling guidelines.
Additional Forms
- Authorization for Non-Legal Guardian Accompaniment – Required when a child is brought in by someone other than their legal guardian for their first visit.
- Happy Dental Visit Tips – A resource for parents or guardians who won’t be attending their child’s appointment, offering guidance on preparing their child for a positive experience.
